Balsam Hill® Rebate – Terms and Conditions

Steps Needed To Claim $50 Rebate From Balsam Hill®:

1. Purchase any tree from a Balsam Hill® that is 6.5 feet or taller from www.balsamhill.com, by phone, or in a Balsam Hill® showroom between November 19, 2008 and December 2, 2008 (the “Promotion Dates”) and download and print a $50 rebate offer from Balsam Hill®.  Trees included are Aspen Estate Fir, Aspen Silver Fir™, Colorado Mountain Spruce, Vermont White Spruce, White River Pine, Norway Spruce, BH Balsam Fir®, BH Noble Fir®, Rogue River Spruce, California Baby Redwood™, Blue Spruce, Denali White, Fraser Fir, Rockefeller Pine, Rocky Mountain Pine, Scotch Pine, Adirondack Evergreen, and Fifth Avenue Flatback. Trees that are on clearance do NOT qualify for this Rebate offer. The participating trees must be purchased online during Promotion Dates.  Rebate submission must be postmarked within 30 days of purchase. 2. Send a fully completed and signed $50 rebate claim request (“Rebate Claim”) containing all of the following: (a) the original completed and signed rebate request form (obtained directly here), with valid name, U.S. street address, phone number, and date of birth; (b) a copy of your printed receipt containing your order number and customer number; and (c) the original white UPC code sticker located on your box, to:

   All Rebate Claims must be postmarked within 30 days of purchase and received at the Balsam Hill® rebate address no later than 2 weeks after the postmarked date.  The $50 Rebate payment will be made in the form of a check.  The name appearing on the Rebate Claim must match the information provided on the online receipt from your purchase.  The Rebate Claim must be signed to be considered complete.   
    
3. Comply with all other terms and conditions set forth below.
    
   Additional Terms & Conditions:  Offer valid in the USA for purchasers 18 years of age or older; must be a legal U.S. resident.  Participating products purchased before or after the Promotion Dates are not eligible for the rebate offer.  Purchase date is determined by date printed on sales receipt. Rebate Claims from Post Office Boxes will not be accepted, and Rebate Claims with invalid, undeliverable and non-U.S. mailing addresses will be rejected.  Balsam Hill® is not responsible for printing errors or for lost, late, mutilated, misdirected or postage due mail.  Rebate Claims must include all components listed in Section 2 above.  Incomplete or illegible Rebate Claims will not be honored.  Omission of any necessary information will result in a letter notification of ineligibility and will require re-submission.  Rebate rights cannot be assigned or transferred and this offer is void where taxed, restricted or prohibited by law.  Offer not available to manufacturers or resellers. No substitutes or rainchecks.  Keep copies of all materials submitted because Rebate Claims become property of Balsam Hill® and will not be returned.  Fraudulent submissions to obtain multiple rebates could result in federal prosecution under mail fraud statutes (Title 18, USC Sections 1341 & 1342).  Balsam Hill® reserves the right to substantiate Rebate Claims and request additional identification and/or documentation.  Rebates paid on products subsequently returned to a retailer are subject to reclamation by Balsam Hill® or its agent.  Rebate checks are void if not cashed within ninety (90) days of issuance and will not be reissued.  Rebate checks will be mailed within twenty (20) weeks of receipt of properly completed submission.  If you have not received your check within twenty-two (22) weeks of this date, please send an inquiry to Balsam Hill® Rebate, 751 Laurel Street, #709, San Carlos, CA 94070-3113. Only actual purchasers of the product may participate in this rebate program. Offer not valid for employees of Balsam Hill®.  Offer is not valid in New York or North Carolina.  Offer not valid in conjunction with corporate or bulk discounts.  The rebate form has no cash value.  This promotion is governed by the laws of the State of California for the resolution of all claims and disputes.  If any part of these terms and conditions is deemed to be invalid or otherwise unenforceable or illegal, the balance of the terms and conditions shall remain in effect and shall be construed in accordance with its terms as if the invalid or illegal provision were not contained herein.  All trademarks are owned by the respective rights holders.
